If a student is to miss a lesson for ANY reason, All That Jazz Music Academy MUST be notified by 12:00 pm (noon) on the day of the lesson, or by 8:00 am on the day of the lesson for Saturday lessons, in order for a make-up lesson credit to be issued. Once issued, a make-up lesson credit will never expire, as long as the student is currently enrolled and tuition is paid up-to-date. Make-up lessons will be scheduled based on teacher and student availability, and in some cases a qualified substitute teacher may be used. New York Music Academy does not issue discounts or tuition credits for missed lesson. If a teacher will be absent and unable to conduct a scheduled lesson, a qualified substitute may be used, or a make-up lesson credit will be issued.
Tuition is due during the last week of each month, for the following month. Tuition is considered late after the first of the month. Payments may be made in person, by phone, on line, or by utilizing our convenient "Auto-Pay" option. All lessons MUST be paid for in advance. We reserve the right to decline any unpaid lessons. Lesson times may be forfeited if not paid for in advance. There is a $20.00 returned check charge for any checks returned by the bank.
The undersigned gives permission to New York Music Academy, its owners and operators to seek medical treatment for the participant if they are not able to reach a parent or guardian in the event of a medical emergency.
We strongly encourage you to utilize our convenient "Auto-Pay" option. Under the "My Account" tab, simply click "e-Payment Schedule", then highlight the "Auto-Pay" option. Under this option, payments are automatically processed on the last day of each month, for the following month's tuition, and any outstanding balances (books, supplies, instruments, unpaid tuition, etc.). To discontinue auto-pay, simply click "manual", in the same drop-down menu. NOTE: You must have entered credit/debit card or bank info. under the "My Account" tab to pay online, or participate in "Auto-Pay". By highlighting "Auto-Pay" in the e-Payment dropdown under the "My Account" tab, you are authorizing us to charge your card/bank account for any unpaid balances on the designated day. To discontinue "Auto-Pay", simply select "manual" under the "e-Payment Schedule" drop-down.
